1. How do I access Employee Navigator?

3. I am changing from part time to full time. What happens?

Your pay may change and you will be eligible for benefits including medical, dental, vision, and retirement. See HR for more information.

4. I am changing from full time to part time. What happens?

Your pay may change and you will no longer have the availability of benefits including medical, dental, vision and flexible spending account (FSA). All of your benefits will stop at the end of the month in which you change status. Your FSA will stop immediately, but you may be able to use the funds until the end of the plan year by using a manual claim form or by online submission.

5. What is PMAM and why do I keep getting emails from them?

PMAM is a training software that is being used by our HR office. We push out training from time to time in order to stay compliant and up to date with certain aspects of City government. Sometimes it is just something to read and sometimes it is a video that is to be watched. The videos are best watched on Google Chrome web browser. Thank you for keeping up to date. We try to give you plenty of time to perform these tasks. You usually have about a month to finish the training.

6. I have been injured on the job. What do I do next?

Contact your supervisor ASAP and if you need to, seek appropriate medical attention.  

If you are injured on the job for the City of Jonesboro, please use the forms below.  You will need to complete the accident form below.  Your supervisor will need to complete the second page.  There are 2 pages total to complete.  

If you are injured and need medical attention, you will also need to complete the "N" Form that is below also. You must fill out the first two pages and sign the 4th page. 

When all necessary forms are completed, the accident form and the "N" form, forward all forms to HR.  We will then input the information onto the ARML website and file the claim for you.  If you receive ANY bills from seeking medical attention, bring those bills to HR and we will contact the medical provider and give the proper information to the provider.  If you are given a prescription, please use Soo's Drugs on Nettleton Ave.  They will call us and confirm the Workers Comp information.  

7. How do I change my Federal and State withholdings?

You can print the FED WH (W-4) and the state withholding form. Complete and submit the form to HR.

8. When I retire and turn 65 with medicare, do I get to keep my insurance through the city?

The answer is Yes you can keep it. You will be able to use it as a supplemental insurance. You will have to contact the insurance carrier for pricing of premium amounts.

9. Is my dependent still covered if they are aged 65 or over?

Yes, dependents can still remain on the plan until the employee turns 65. However, it might be more fiscally advantageous for said dependent to go to Medicare.

10. How long does my Short Term Disability (STD) last?

Short Term Disability will last up to 6 months from the date you are initially off work.  After it stops, if you have LTD it will start where the STD stopped.  

11. How long does Long Term Disability (LTD) last?

Long Term Disability (LTD) will last until the age of 65 as long as the premiums are paid. 

12. What is a deductible?

The amount you're responsible for paying for covered medical expenses before your health insurance plan begins to pay for covered medical expenses each year.

13. How do I add a family member to my insurance?

The general rule is usually you can only make changes to benefits during the annual "Open Enrollment Period" each December. There are exceptions to this rule when you have a "qualifying event". Examples of a qualifying event is a birth of a child, marriage, or divorce, etc. Contact your HR representative for further information.

14. When does my insurance coverage start?

Coverage starts the first month after 30 days of employment. Premiums start one month before effective date of benefits.

15. What holidays do city employees get off?

16. I have left the company, when does my coverage end?

The month in which an employee was terminated

17. Whom can I put on my insurance?

Typically speaking, an employee's child, a qualifying child, and a qualifying relative are considered dependents.

18. What is a qualifying event?

Some examples of qualifying events include: Change in marital status, birth or adoption of a child, death of a covered dependent, loss of eligibility status by a covered dependent, change in employment status that affects eligibility for coverage

19. I have lost my TASC (FSA) card, How do I order a new one?

21. What is the maximum out of pocket expense for an individual and/or a family

22. How do I update my personal information with the Human Resources department

23. How do I file a Long-Term Disability claim?

Fill out the Long Term disability claim form.  Submit to HR. We have to fill out a section also.  We will fax it to the proper people and submit it for you. 

24. How do I file a Short-term Disability claim?

Fill out the Short-term disability claim form.  Submit to HR. We have to fill out a section also.  We will fax it to the proper people and submit it for you. 

25. What is myBlueprint?

My Blueprint allows a member to view their health plan information. On the site a member can access benefits and coverage, claims, pharmacy information, and many of policy details.

26. Is your USABLE Life Insurance Portable When You Leave?

Yes it is. You will need to fill out the "Application for Portability of Group Term Life" document (see "Important Forms" section of this webpage).
